ONE OR MORE OF THE FIVE COOLING FAN BLADES MAY HAVE BEEN IMPROPERLY RIVETED TO THE FAN SPIDER. THIS COULD ALLOW THE BLADES TO SEPARATE FROM THE FAN DURING ENGINE OPERATION.
